2018 / 19 League Registration Forms & New Player Signing On Forms

A minimum of 8 registrations for 5v5, 10 for 7v7, 12 for 9v9 and 14 for 11-a-side must be with me no later than 21st July 2017.

Teams will not be accepted into the League if the minimum number of player registrations are not submitted by this date ie 21st July 2018.

Please note that only recent photographs for players will be accepted. Photographs sent in and are already on the system will be sent back and this will delay the registration.

Please can you ensure the managers/parents understand that photo's have to be recent.

Registration of players will only be completed when all documentation is received. Players who have played in the Junior League before only need to send in a signed registration form and recent photo, players who are new to the Junior League need to sent a signed registration form, recent photo and a copy of birth certificate or passport. Do not send original copies of birth certificates. Registration fees (£2.50 per player) have to also be send at the time of submitting the registration form.

Please do not include or send in any player on the registration form if the birth certificate (where needed) and photo is not included.

All registration forms should be signed by the secretary and please make sure the team name is included on the form I received a large amount of registration forms last season without this information which caused delay in registering the players as I had no idea which team there were to be registered to.

If I do receive any incomplete registration forms they will be return to you and registration will be delayed.

Can you please ensure that parents understand that signing the form allows HJFL to keep there information for the season unless their child leaves the competition in which case it will be deleted.
